COST STRUCTURE: The DAHLC encourages utilization; the more you use the center, the less you will pay the next period. Members may earn one incentive visit per day. Incentive visits are re-assessed every three months and discounts are applied to the subsequent three-month period.
Payroll Deduction: Mayo employees with 0.5 FTE or greater may be eligible for payroll deduction each pay period. Spouses and Adult Dependents of this membership type can be deducted from the employee’s payroll. Everyone starts off paying $16.16/pay period for the first quarter (3 months). Discounts are applied based on facility utilization for the next quarter:
- 0-14 visits = $16.16/pay period
- 15-29 visits = $13.85/pay period
- 30+ visits = $11.54/pay period
If Employee & Spouse join, both would have the same cost structure ($16.16/pay period/person).
Pre-pay: A three-month prepaid membership is available for those who are eligible for membership but are not 0.5 FTE or greater. Membership renewal is required to receive discounted rates. Everyone starts off paying $105 for the first three months. Discounts are applied based on facility utilization for the next three months’ payments:
- 0-14 visits = $105/3 months
- 15-29 visits = $90/3 months
- 30+ visits = $75/3 months
