How do I cancel my membership?

DAHLC-2547We're sad to see you go, but understand this may not be the right fit for you at this time. To cancel your, your spouses or your dependent child's membership from payroll deduction, the Mayo employee will need to stop at the downtown facility to sign a form that will authorize us to cancel the membership(s) and make a change with his/her payroll. Please also turn in any DAHLC-issued materials (including DAHLC parking tags and non-employee access cards) at the time of cancellation. Pre-paid membership dues are non-refundable and non-transferable.
