DAHLC COVID-19 Vaccination Requirement Update

Beginning Sept. 1, the Dan Abraham Healthy Living Center will require proof of COVID-19 vaccination to use the facility. This change aligns with many fitness facilities across the nation. This temporary change will be reassessed based on the evolving COVID-19 situation.

The additional step of requiring proof of COVID-19 vaccination joins other precautionary measures in place, such as routinely cleaning equipment and appropriate social distancing. We are confident we will continue to provide a safe environment for fully vaccinated members.

What members need to do
What you need to do in response to the new vaccine requirement depends on whether you were fully vaccinated for COVID-19 by Aug. 30, and this COVID-19 vaccination information was documented with Occupational Health Services:

  • Your information is documented with Occupational Health Services. Your information will be automatically uploaded into the Dan Abraham Healthy Living Center's member management system. You will be able to check in and use the facility immediately. You do not need to take any further action.
  • Your information has not been documented with Occupational Health Services. You should complete and submit documentation using the Dan Abraham Healthy Living Center COVID-19 Vaccine Verification form. Once submitted, you will receive a verification email that the Dan Abraham Healthy Living Center has approved your COVID-19 vaccination status. After receiving this verification email, you will be able to check in and use the facility immediately.

What if you cannot meet the new vaccine requirement?
If you cannot meet the center's new vaccine requirement for members, you can change your membership status using the Dan Abraham Healthy Living Center Membership Cancel/Freeze/Unfreeze form.
