Three steps to begin your membership at the
Mayo Clinic Dan Abraham Healthy Living Center (DAHLC)
Step 1: Verify Eligibility: Because the DAHLC is an employee benefit, it must follow the IRS tax benefit guidelines. Click to read more and verify your eligibility. Proof of COVID-19 vaccination is required in order to use the facility.
Step 2: Complete member orientation - watch the orientation video below. If you have any questions, please call the front desk at 266-4688.
Step 3: Complete Membership Agreement. You will receive an email confirmation when your membership is active. If you select the prepaid membership option, please call the front desk 266-4688 or stop at the front desk on your first visit to the facility to make payment.
